Delta CONNECT is an advanced emergency and remote calling service that provides help and support when you need it, 365 days a year.

It includes the latest technology-based care packages, tailored to your individual needs, as well as proactive phone calls to help identify any potential health and wellbeing issues as early as possible.

And if an emergency occurs at home, a 24-hour community response team will be immediately dispatched to provide assistance.

Margaret Williams signed up to CONNECT after returning home on New Year’s Eve last year. He couldn’t get up and couldn’t pick up the phone to call for help.

She had been lying on the floor for more than 15 hours when her daughter arrived on New Year’s Day and called an ambulance. She was hospitalized because she developed pneumonia from lying on the floor for too long.

Since CONNECT, Ms Williams has suffered a few more falls, but the response team arrived within 30 minutes, helped her into the chair safely and even made her a cup of tea before they left.

Mrs Williams said: “They care a lot and I don’t think I would stay in my flat without them. Knowing that they can help me if I fall gives us a lot of peace of mind.

“I love people calling me to see how I’m doing too. “Both of my daughters work full time, and although we talk every night, it’s great to spend the day with the lovely Delta team.”

Delta CONNECT is changing the way social care is delivered in Wales

Flexible maintenance packages include a red button that communicates with the monitoring center and provides 24/7 support at the push of a button.

Depending on needs, fall detectors, GPS tracking devices, door sensors, medication dispensers and other useful equipment can also be provided.

Delta Wellness Specialists can host weekly, monthly or quarterly proactive wellness calls and help people stay connected with family and friends by supporting them with additional support and wellness activities.

The Response Team is ready to support you in crisis situations for non-medical emergencies such as non-injury falls and other social safety needs, giving you and your loved ones peace of mind knowing that help is always available if you need. .

Funded by the Welsh Government’s Regional Health and Social Care Integration Fund (RIF) through the West Wales Health Partnership Board, Delta CONNECT is the first of its kind in Wales.

It is changing the way social care is delivered, introducing a model of self-help and proactive care to improve wellbeing and help people stay independent for longer.

Delta CONNECT is currently FREE for the first three months for residents of Carmarthenshire, Ceredigion and Pembrokeshire, and from £28.46 per month thereafter. There is no obligation to continue the service after the free period.
