Subtle changes in a person's face, from puffy eyelids to a grayish complexion, may be among the earliest warning signs of kidney disease, according to health researchers, sometimes appearing long before standard blood tests detect a problem.

Roughly 30 million Americans, about one in seven adults, are living with kidney disease, and nine out of ten do not know they have it. The kidneys are fist-sized organs that filter waste and excess fluid from the blood and expel it as urine. When they are damaged, often by diabetes or high blood pressure, their filtering units become scarred and lose the ability to clear waste, a condition often dubbed a "silent killer" because it rarely produces symptoms in its early stages.

By the time symptoms such as fatigue, nausea or swelling in the legs appear, kidney function may already be significantly reduced. Clinical research and centuries-old medical traditions alike point to the face as a revealing indicator of kidney health, sometimes flagging trouble before lab results do.

Pale or Sallow Complexion

Healthy skin gets a rosy tone from oxygen-rich blood near the surface, but that glow can fade as kidney function declines. Healthy kidneys produce a hormone called erythropoietin, or EPO, which signals the bone marrow to make red blood cells. When kidney function drops, EPO production falls too, leading to anemia, a shortage of oxygen-carrying red blood cells.

Because skin is the body's largest organ, it is often the first place this shortage becomes visible, losing its healthy pink tone for a pale, dull or sallow look. Traditional Chinese Medicine identified the same pattern centuries ago, describing a dull or sallow complexion as a sign of "kidney qi deficiency," or waning core vitality.

Puffy Eyelids and Facial Swelling

Puffiness around the eyes and general facial swelling are among the best-supported facial signs of kidney trouble. Healthy kidneys keep proteins and electrolytes balanced so fluid stays inside blood vessels, but damaged ones can lose the ability to filter out excess sodium and water, causing fluid to collect in the tissues.

Damaged kidneys can also leak a protein called albumin into the urine. Albumin normally holds fluid inside blood vessels, so when it is lost, fluid escapes into surrounding tissue. The eyelids, with their thin, loose skin and little underlying muscle, are especially vulnerable, which is why puffiness often appears there first, particularly in the morning after fluid pools around the eyes overnight. When swelling is more widespread, the whole face can look rounder or fuller, and pressed skin may temporarily indent, a classic sign of edema.

Dull and Darkened Complexion

A dull, ashen or darkened complexion is another facial sign grounded in research. As kidney function declines, waste products such as urea and creatinine build up in the blood, a condition known as uremia. Because skin is highly vascular, it often reflects that buildup before other organs show trouble.

The waste compounds can interfere with melanin production. Elevated levels of a hormone called beta-melanocyte-stimulating hormone push melanocytes, the pigment-producing cells, to work overtime, leaving skin with a grayish or muddy tone, and in some cases a bronze or yellowish hue. Studies show pigmentary changes affect anywhere from 25 to 70 percent of people with chronic kidney disease, rising to roughly 65 percent among dialysis patients and about 37 percent among those not yet on dialysis.

Abnormal Lip Pigmentation

Healthy lips are typically pink or reddish, reflecting good circulation, but kidney failure can change that too. Abnormal lip pigmentation, ranging from brown patches to overall darkening, is one of the more common oral signs of chronic kidney disease, affecting roughly 40 percent of end-stage patients on dialysis, and as many as 48 percent in some populations, studies show.

The change stems from the same hormone buildup seen in skin darkening. Healthy kidneys normally filter beta-melanocyte-stimulating hormone from the blood, but when function declines, it accumulates and stimulates melanocytes in the lips and mouth. Traditional Chinese Medicine also long described dark or purplish lips as a sign of "blood stasis," or poor circulation, an observation that lines up with the modern understanding of toxin buildup.

Oily Cheeks and Acne

Acne and oily skin are common and usually unrelated to kidney health, but Traditional Chinese Medicine treats the cheeks and chin as reflective of overall kidney and reproductive health, and practitioners sometimes look to persistent, recurrent or treatment-resistant breakouts in these areas as a clue.

The scientific link is far weaker. Hormonal shifts tied to chronic disease, including changes in androgen levels, can influence oil production, but these are not specific to kidney disease and can also stem from stress, diet, polycystic ovary syndrome or genetics. One indirect connection stands out: chronic kidney disease often causes toxins to build up in the blood, triggering systemic inflammation that can worsen acne in people already prone to it.

Steps to Protect Kidney Health

Much of the damage to kidneys can be prevented or slowed, and diet plays a major role. Plant-based diets have been linked to a lower risk of chronic kidney disease and slower progression. In large studies, people who ate predominantly plant-based diets had roughly 20 percent lower levels of albuminuria, a key marker of kidney damage, and 30 percent lower odds of developing kidney disease than those who ate more animal protein.

Experts recommend keeping sodium intake under 2,300 mg a day and limiting added sugar, since processed foods often contain hidden phosphorus and potassium additives that can strain damaged kidneys. Regular exercise also helps. The CDC recommends 150 minutes of moderate-intensity activity a week plus two days of muscle-strengthening exercise, and a network meta-analysis of 25 randomized trials found that moderate exercise can meaningfully slow the decline in estimated glomerular filtration rate, or eGFR, the key measure of kidney function. Even 30 minutes of activity five times a week has been linked to better outcomes.

High blood pressure is the second leading cause of kidney failure in the US, making it important to keep blood pressure below 140/90 mm Hg, while people with diabetes, the most common reason for needing dialysis or a transplant, are advised to keep tight control of their blood sugar. Common over-the-counter pain relievers such as ibuprofen and naproxen should also be used with caution, especially by people with kidney disease, diabetes or high blood pressure, and anyone taking them regularly for chronic pain should talk to a doctor about safer alternatives.
