Full-fat dairy does not cause weight gain, study finds

A University of Toronto study found that eating three daily servings of full-fat dairy does not cause weight gain or raise blood cholesterol levels.

Eating three daily servings of full-fat dairy does not cause weight gain or raise blood cholesterol levels, according to a 12-week study by Canadian researchers. For decades, health guidelines have urged consumers to avoid saturated fat and select low-fat or fat-free options for heart health and weight management, but the new trial suggests that advice may be outdated.

The study, published in the Journal of Nutrition, was conducted by nutrition scientists at the University of Toronto in Ontario and Mount Saint Vincent University in Halifax. Researchers tracked 74 overweight and obese adults to determine how different levels of dairy fat impact body weight, blood pressure, lipid profiles, and nutrient intake.

Participants were randomly assigned to one of three specific dietary groups for the 12-week period. One cohort followed a calorie-restricted diet containing no more than one daily serving of low-fat dairy. A second cohort consumed three daily servings of full-fat dairy while maintaining neutral calorie intake, and a third group consumed three daily servings of full-fat dairy with no calorie restrictions.

Results showed that neither group eating three daily servings of full-fat dairy experienced significant weight gain over the three-month trial. Furthermore, participants in the unrestricted group who ate full-fat dairy without calorie limits recorded a slight decrease in hip circumference.

Both full-fat dairy groups recorded improvements in cardiovascular health, with systolic blood pressure dropping by an average of 2.72 mmHg. Systolic blood pressure measures the pressure inside blood vessels when the heart contracts. The low-fat calorie-restricted group recorded a slightly larger average drop of 4.25 mmHg, though researchers noted the difference between groups was not statistically significant. Lead investigators concluded that while full-fat dairy did not outperform low-fat dairy on this metric, it still yielded clear blood pressure benefits.

Blood testing showed no meaningful differences in cholesterol levels among any of the three groups. Total cholesterol, low-density lipoprotein (LDL) cholesterol, high-density lipoprotein (HDL) cholesterol, and blood sugar levels remained stable throughout the study. Triglyceride levels spiked temporarily at week four in participants, but returned to normal baseline levels by week 12.

Lead author Dr Harvey Anderson, a nutrition scientist at the University of Toronto, confirmed that whole dairy consumption did not produce negative metabolic indicators. "Those who had three servings of dairy, you don't have adverse levels of blood cholesterol or lipids or evidence of insulin resistance," Anderson said in a statement.

The dairy matrix hypothesis

Researchers said the findings support the dairy matrix hypothesis. The theory posits that dairy possesses a unique physical structure that slows digestion and reduces the absorption of saturated fat, preventing dairy fat from affecting cholesterol in the same manner as fat from processed foods.

Anderson noted that dairy contains a complex structural bond that affects how nutrients interact in the digestive system. "With dairy products, it's got two proteins - casein and whey - that are bound together with fat and with nutrients mixed in," Anderson said. He added that this complex structure ensures a slow and steady release of nutrients, offering health benefits beyond the sum of isolated nutrient components.

Nutrient intake was the area where full-fat dairy groups demonstrated the strongest advantages over the low-fat diet. Participants who consumed three daily servings of full-fat dairy significantly increased their intake of protein and calcium compared to those on the low-dairy calorie-restricted diet.

Daily calcium intake in the high-dairy groups averaged more than 1,100 mg per day, exceeding the standard recommended daily intake of 1,000 mg. In contrast, the low-dairy group averaged 760 mg per day, falling short of daily dietary requirements. Protein intake also rose substantially among full-fat dairy consumers, while the low-dairy group experienced a slight decline in protein intake.

Dietary guidelines and nutritional needs

Vitamin D intake improved among participants in both full-fat dairy groups, although total intake remained below recommended targets across all trial cohorts. The research team noted that Canada has since doubled its mandatory vitamin D fortification requirements for fluid milk, a regulatory change expected to boost dairy as a dietary source of the vitamin.

The findings align with updated dietary guidance in the United States. The Department of Agriculture (USDA) recently revised its national food pyramid under Health Secretary Robert F Kennedy Jr. to emphasize full-fat dairy, protein, and healthy fats, marking a major policy shift from previous recommendations.

However, researchers cautioned that the findings do not mean full-fat dairy consumption is entirely unrestricted. Current USDA dietary guidelines continue to cap saturated fat intake at 10 percent of daily calories, which equals approximately 22 grams of saturated fat on a standard 2,000-calorie daily diet.

Consuming three daily servings of whole dairy can approach that upper threshold quickly. Combining one cup of whole milk, one serving of whole-fat yogurt, and one serving of cheese accounts for approximately 17 grams of saturated fat before any other food is consumed during the day.

Nutritionists note that dietary fats are necessary for bodily function, and whole dairy delivers healthy fats that facilitate the body's absorption of fat-soluble vitamins, including vitamins A and K. Whole dairy also contains higher amounts of omega-3 fatty acids and promotes satiety, helping individuals feel full longer than reduced-fat options.

The study authors concluded that deciding between whole and low-fat dairy depends on individual nutritional requirements. Reduced-fat options remain suitable for individuals looking to restrict saturated fat, while full-fat dairy provides benefits for those needing to increase protein, calcium, or vitamin D intake. "Keep it simple, eat a variety of foods and not too much of anything," Anderson said.
